Skilled Nursing, Home Health Aide, and Physical Therapy

We provide a full range of home health services - skilled nursing, home health aide, and physical therapy.  These services are designed to help improve your overall function, keep you from having unnecessary hospital and emergency room visits, and teaching you the skills you need to help manage your ongoing medical conditions.

Skilled nurses (RN and LPN) are available for:

-Assessment and teaching on medical conditions

-Medication teaching and management

-Chronic disease management (for conditions such as diabetes or heart failure)

-Complex wound care, including wound vacs

-Infusion and IV therapy

-Management of feeding tubes

-Ostomy and catheter care

Home health aides are available to provide assistance with activities of daily living, such as bathing and dressing.  Our home health aides receive ongoing supervision and training by a registered nurse.

Physical therapists are available to help restore your everyday functioning, by focusing on things such as strength and endurance.  Their services are invaluable after a knee or hip replacement, stroke, or weakness following a prolonged hospital stay.
